The Immortal's Head is an Epic Back Bling in Fortnite: Battle Royale, that can be purchased in the Item Shop with Omni-Man for 1,500 V-Bucks or with the Guardians of the Globe Bundle for 2,800 V-Bucks. The Immortal's Head was first released in Chapter 4: Season OG and is part of the Guardians of the Globe Set.
